Answer
How do I fill out the Data Profile for a department or program within a larger organization?

In the Cultural Data Profile (CDP), a parent organization refers to the larger entity that houses a department or program filling out the CDP. You can indicate that you are a program or department within a parent organization when you setup your organization's account by selecting that option in the Organization Type question.  This will tailor the CDP to your organization, department or entity, and prompt you to enter the name and EIN of your parent.

Should I fill out the CDP this way?

If your parent organization is 100% culturally focused, consider filling out a CDP for your entire parent organization, rather for only the smaller entity under a parent. This may make the data entry process easier.

Enter your revenue correctly

In the Revenue section, you will see a line in the Contributed Revenue sub-section called Parent Organization Support.  Use this line if you receive monetary funding or in-kind contributions directly from the larger organization.
